Church Pharmacy is a registered pharmacy regulated by the General Pharmaceutical Council (GPhC). Our pharmacy services are provided in accordance with GPhC standards and, where applicable, NHS contractual requirements.
2. What this Website is for
This Website provides:
- Information about NHS and private pharmacy and clinical services available at our Weybridge premises;
- An online facility for booking appointments for these services;
- General health and travel information.
No payment is taken via this Website. Where charges apply to private services, payment is made in person at the pharmacy at the time of your appointment. NHS services are provided in accordance with NHS terms and are free of charge to eligible patients (with the exception of any applicable NHS prescription charges).
3. Eligibility and use
By using this Website you confirm that:
- You are at least 18 years of age, or you are using the site with the consent of a parent or guardian;
- The information you provide when booking an appointment is accurate, complete, and relates to you (or to a person on whose behalf you are authorised to book);
- You will use the Website only for lawful purposes and not in any way that damages, disables, or impairs the Website or interferes with another user’s use of it.
4. Booking appointments
Our booking platform. Online bookings are taken through Cliniko, a third-party booking and practice management platform operated by Red Guava Pty Ltd. When you make a booking, your details are submitted to Cliniko and stored in our practice management system. Cliniko acts as a data processor on our behalf and processes your information in accordance with our instructions and applicable data protection law. Booking a request, not a guaranteed appointment. When you submit a booking through the Website, you are requesting an appointment. A booking is only confirmed once you receive written confirmation from us (by email or otherwise).
Service suitability. Many of our private clinical services (including but not limited to vaccinations, travel health, weight management, blood testing, and minor ailment consultations) are subject to clinical suitability. Following your booking, our pharmacist or clinician may need to assess whether the service is appropriate for you. We reserve the right to decline or modify a service where, in our professional judgement, it is not clinically appropriate.
Cancellations and rescheduling. If you cannot attend an appointment, please notify us as early as possible by telephone or email. Repeated non-attendance without notice may result in us declining future bookings.
Lateness. If you arrive significantly late for a booked appointment we may need to reschedule it to avoid disrupting other patients.
5. NHS services
Where you book an NHS service through this Website (for example NHS-funded vaccinations or Pharmacy First consultations), the service is delivered under the terms of our NHS contract. NHS-specific rules — including eligibility criteria, record-keeping, and information sharing with your GP and other NHS bodies — apply in addition to these terms. Booking via this Website does not alter your rights or entitlements under the NHS.
6. Private services
Private services are subject to the price displayed at the time of booking or quoted to you at the appointment. Prices may change from time to time but the price applicable to your appointment will be the one confirmed to you before the service is provided. Payment for private services is taken at the pharmacy.
A private service does not create a doctor-patient relationship and does not replace the care provided by your GP. We may recommend that you consult your GP or another healthcare professional where appropriate.
7. Medical information and advice
Information on this Website is provided for general guidance only and does not constitute medical advice. It is not a substitute for consultation with a qualified healthcare professional. If you have a medical concern, please consult your GP, NHS 111, or attend our pharmacy for a face-to-face discussion. In a medical emergency, call 999.
While we take care to ensure information on the Website is accurate and up to date, we do not warrant that it is complete, current, or free from error.
8. Your information and privacy
When you book an appointment we collect personal information (including health information where relevant) to provide the service safely and to maintain proper clinical records. The way we handle your personal data is described in our Privacy Policy, which forms part of these terms.
In summary:
- We process your data in accordance with UK GDPR and the Data Protection Act 2018;
- We use Cliniko (operated by Red Guava Pty Ltd) as our booking and practice management platform; your booking information is processed and stored through Cliniko on our behalf;
- We share information with NHS bodies (including your GP where appropriate, and Public Health England / UKHSA where required) when delivering NHS services or where we are under a legal or professional duty to do so;
- We retain clinical records in line with NHS and GPhC retention requirements;
- You have rights of access, correction, and (in some cases) erasure of your data — see the Privacy Policy for details.

All content on this Website — including text, graphics, logos, images, and the arrangement of the same — is owned by Church Pharmacy or its licensors and is protected by UK and international copyright and trademark law. 15. Governing law and jurisdiction
These terms, and any dispute or claim arising out of or in connection with them or with your use of the Website, are governed by the laws of England and Wales. Any dispute will be subject to the non-exclusive jurisdiction of the courts of England and Wales.

- The General Pharmaceutical Council (GPhC) — for concerns about pharmacy practice or pharmacy professionals;
- NHS England or your local Integrated Care Board — for concerns about NHS-funded services;
- The Parliamentary and Health Service Ombudsman — where an NHS complaint remains unresolved.
