Day 2/8 FAQ’s

A booking code is a unique reference code, which is required for UK government passenger locator forms.

Your booking code will be shown in a confirmation email which is sent after you have made your purchase. The number is included (see below) within the detail of the product purchased.

Please remember that you will need to enter your booking reference number when you register your kit for Day 2 and Day 8 URN registrations.

You will need to take a COVID-19 test on or before Day 2 / Day 8 of your quarantine period. The day of arrival in England will be treated as day 0.

It is important to note the arrival time of your flight when taking your Day 8 test. You must take your swab after this time on your Day 8 for the timing of the test to be acceptable. For example, if you arrive at 9pm on the 1st, you must take and register your test after 9pm on the 9th for your Day 8 test.

To be clear, you must take and register your swab a full 192 hours after arrival in the UK for the system to recognise your Day 8 registration.

You need to take a COVID-19 test on or after day 8, unless you got a positive result from your day-2 test.

A booking reference number is a unique personalised number, which is required for government passenger locator forms.

If you fail to register both kits ie you only register one URN, you will not receive your results for both kits. Please remember to keep a note of your URN numbers as it will be needed to obtain your results.

It is the customers responsibility to follow the instructions provided in the sample kits.

You will need to enter details such as your URN, booking reference, name, email, DOB, phone number, address, travel details, passport number, date & time sample collected.

Test to release / Day 5 FAQ’s

Please note that under UK Government regulation, Test to Release kits can only be despatched prior to Day 5 of quarantine. These are available for UK delivery only. Test to release kits CANNOT be taken any earlier than Day 5. Despatch of day 5 will be based upon the Arrival Date (back into the UK) that is provided when order is placed.

General FAQ’s

Sample collection kit orders are despatched Monday – Saturday.

We recommend you place your order well in advance of when you need it.

Once your order has been accepted by Church Pharmacy an order confirmation email will be sent out to the email address used at time of order placement.

Orders cannot be amended or cancelled once the order has been accepted by Church Pharmacy.

Expected delivery is 2-4 working days once order confirmation is sent. The arrival date recorded as part of the ordering process has no impact on delivery schedule except for Day 5 (Test to release) orders.

For Day 5 (test to release) orders, theses will only be shipped prior to day 5 of quarantine for arrival on Day 5, as per government testing guidelines.

When the products you ordered despatch, an email and/or text message will be sent out by the courier to confirm delivery schedule. If there are any changes regarding delivery, it is the customers responsibility to communicate with the courier once courier has taken receipt of the order.

Once the order is with the courier all delivery queries must be directed to the courier, using the contact information provided in the email/ text communication at despatch.

Each sample collection kit contains instructions for use. It is the user’s responsibility to ensure the instructions are read in full prior to engaging in the sampling process.

Registration is REQUIRED as part of the sampling process. It enables Church Pharmacy to send the results of testing to you. Without registration, results will not be provided.

Once the sampling process has occurred, registering the URN is necessary. Follow the instructions provided with the sample collection kit to carry registration out.

Your URN is your Unique Reference Number.

Your URN is found within your COVID-19 Sample Collection Kit on pre-printed labels.

You must use your URN to register your kit

You must always keep this URN safe/ do NOT lose the URN

You should receive an email confirming your registration was successful. If you are unsure if your registration was successful, please try to register again as the site will not allow you to register the same URN twice.

Using the instructions provided with the COVID-19 sample test kit please ensure that the test is packed correctly.

This will depend on the service you ordered for.

Our same day service – result turnaround is up to midnight of the day same of sample collection.

For our 12-24 hours result turnaround service you will receive your results within 48hrs

For our self-swab sample, you will receive your result within 48 hrs of sample receipt at our partner laboratory.

If you have REGISTERED your sample online and sent the sample back to our laboratory (Please view how do I return my COVID-19 sample test kit), you will be notified when the physical sample has arrived at our laboratory.

Where required, you will receive your results via email, as well as a certificate of your results to use whilst travelling. In order for your certificate to be correct, please ensure the information used during the registration process matches the details on your passport.
